The incidence of adverse events was 79.1% in the eplerenone group and 79.2% in the placebo group. The incidence of adverse drug reaction was 24.3% in the eplerenone group and 18.8% in the placebo group. The incidence of serious adverse events was 23.0% in the eplerenone group and 20.8% in the placebo group. Thus, there was no obvious difference in the incidence of adverse events or adverse drug reaction between the eplerenone and the placebo groups. Two patients in the eplerenone group died, while none in the placebo group did; however, both deaths were considered as non-drug-related by the investigator.
The incidence of composite endpoint (cardiovascular death or first re-hospitalization due to cardiovascular disease within 6 months) as primary endpoint was 19.5% in the eplerenone group and 17.2% in the placebo group. In the primary analysis, using patient classification (SBP > 140 mmHg in acute heart failure patients except after acute myocardial infarction / SBP ? 140 mmHg in acute heart failure patients except after acute myocardial infarction / heart failure patients after acute myocardial infarction) as a stratification factor and first occurrence/recurrence as a covariate, the hazard ratio of the eplerenone group to the placebo group (95% CI) was 1.09 (0.642, 1.855). Therefore, there was no consistency of the primary endpoint between the EPHESUS trial and the EARLIER study.
Regarding the secondary efficacy endpoints, the hazard ratio (95% of CI) for incidence of "cardiac death or re-hospitalization due to heart failure" (within 1 month or 6 months), "re-hospitalization due to heart failure" (within 1 month or 6 months) and "nondrug treatment for heart failure (implantation of CRT-D or cardiac support device)" was less than one, but not statistically significant.
The EARLIER study, comparing the efficacy of eplerenone with placebo in the treatment of acute heart failure, failed to achieve the predetermined criterion for consistency with the EPHESUS trial. However, the hazard ratio (95% CI) of "re-hospitalization due to heart failure and cardiovascular death" or "re-hospitalization due to heart failure" within 6 months in the eplerenone group was 0.55 (0.213, 1.434), suggesting that eplerenone prevents re-hospitalization due to heart failure. Moreover, plasma BNP, left ventricular ejection fraction, left ventricular end-diastolic dimension, left ventricular end-systolic dimension and E/e' numerically improved in the eplerenone group compared with the placebo. The safety of eplerenone in patients with AHF was validated in the EARLIER study.

The incidence of acute heart failure has been steadily increasing as manifested by various factors such as the aging society, westernization of life-style, as well as an increased incidence of chronic heart failure. The current treatment of acute heart failure is mainly intended for rescue treatment and symptomatic improvement, through the improvement of cardiac hemodynamics using diuretics, vasodilators, and cardiotonics. On the other hand, treatment objectives for chronic heart failure have gone through a paradigm shift in the past 20 years, that is, from therapies with emphasis on symptomatic improvement to those intended to improve prognosis while offering organ protection. The same paradigm shift for the treatment of acute heart failure, that is, to the improvement of prognosis accompanying organ protection, is also demanded, as evidenced by recent reports suggesting the needs for development of such therapies. Aldosterone, the signal that comes in the end of the renin-angiotensin-aldosterone-system, is catching attention as a target agent associated with organ damages during acute phase of heart failure. For chronic heart failure, aldosterone control has been shown to remarkably improve prognosis, for which, clinical studies in patients with chronic heart failure intended for additional indication have been on-going in Japan. There have also been study reports demonstrating that patients with acute heart failure have a high blood aldosterone concentration, and that a high blood aldosterone concentration is associated with poor prognosis. These findings suggest that the control of aldosterone effect during the acute phase of heart failure may provide remedy with a new concept for the treatment of acute heart failure offering to protect the heart and other organs, improve prognosis of patients with acute heart failure, and furthermore to prevent acute exacerbation of chronic heart failure.
To compare and evaluate eplerenone, a selective aldosterone antagonist, with placebo in terms of whether eplerenone can control the occurrence of the 6-month combined end point (cardiac death or first re-hospitalization due to cardiac disease) after given eplerenone in patients in the acute phase of acute heart failure. Also, to verify the developing tendency of combined end point is consistent with the results of EPHESUS Study. By "consistent", it means that the point-estimate value of hazard ratio in eplerenone arm is smaller than that in placebo arm.
