TEAEs were reported in 2 subjects (100.0%) in Cohort 1, and in 1subject (100.0%) in Cohort 2. In addition, one subject reported possible sequelae of fat-soluble vitamin deficiency in Cohorts 1 and 2 (1/2 subject, 50.0%, and 1/1 subject, 100.0%, respectively). No TEAE related to study treatment, TEAEs leading to death, SAEs, TEAEs leading to dose reduction, treatment interruption, or treatment discontinuation were reported in the study. Except for possible sequelae of fat-soluble vitamin deficiency, no TEAEs of interest (liver decompensation events, liver related events, fat-soluble vitamin deficiency, clinically significant diarrhoea, and bleeding) were reported in either cohort. |

| / | •コホート1の1例では、用量調整期で主要評価項目であるベースラインから24週時までに空腹時血清中胆汁酸濃度が70%以上低下した被験者、又は24週間の投与後に空腹時血清中胆汁酸濃度が70 µmol/L以下に到達した被験者に該当し、その低下は22週時、24週時、及び24週時(平均)で認められた。一方、コホート1の他の1例及びコホート2の1例では、24週までの用量調整期で血清胆汁酸濃度低下の基準への到達は認められなかった。 •48週時までのVisit別の血清中胆汁酸濃度は、コホート1の1例では24週時から48週時(平均)にかけて70 μmol/L以下に到達し、ベースラインから著明に減少した。また、コホート1の他の1例はベースライン欠測のため、ベースラインからの変化量は算出できなかったが、投与期間中の値はいずれも70 µmol/L未満には達せず、経時的に有意な減少も認められなかった。コホート2の1例では48週までの期間を通じてわずかに減少した。なお、48週までの期間では、「ベースラインから48週時までに空腹時血清中胆汁酸濃度が70%以上低下した被験者」の基準に該当する被験者は認められなかった。 •48週時以降の血清中胆汁酸濃度は、コホート1の1例では最終評価時点(112週時)まで70 μmol/L以下を維持し、ベースラインから著明に減少した。他の1例はベースライン欠測のため、ベースラインからの変化量は算出できなかったが、投与期間中の値はいずれも70 µmol/L未満には達せず、経時的に有意な減少も認められなかった。コホート2の1例では、ベースライン時(248.1 µmol/L)と比較して低下傾向を示し、94週時(72.1 µmol/L)で空腹時血清胆汁酸濃度の70%以上低下が認められた。 •48週時まで、また48週時以降のVisit別の痒み評価が改善した被験者の割合(ObsRO、午前+午後)は、コホート1では12から32週時、及び40から48週時、60、70、72、84、94、及び96週時で各50.0%(1/2例)、コホート2では4、8及び16週時、並びに84週時で各100.0%(1/1例)であった。 •コホート1の1例は、身長及び体重は各評価時点で標準成長曲線の値を下回っていたが、BMIはわずかに上回っていた。他の1例は、身長はベースラインから22週時までの期間及び28週時では標準成長曲線の値を下回っていたが、それ以降の評価時点の大部分ではわずかに上回っていた。体重及びBMIは各評価時点で標準成長曲線の値を上回っていた。コホート2の1例は、身長、体重、及びBMIは各評価時点で標準成長曲線の値を下回っていた。 •24週時及び48週時のPGIC(痒み/引っ掻き)は、コホート1の1例では患者A little better及びNo change/保護者Moderately better及びA little better/医師No change及びNo change、他の1例では保護者Very much better及びModerately better/医師Very much better及びModerately betterであった。コホート2の1例では患者No change及びNo change/保護者A little better及びNo change/医師No change及びA little betterであった。 •最終評価時点のPGIC(痒み/引っ掻き)は、コホート1の1例では患者A little better/保護者Moderately better/医師A little better(112週時)、コホート1の他の1例では保護者Very much better/医師Very much better(112週時)であり、改善が認められた。コホート2の1例では患者No change/保護者A little better/医師A little better(96週時)であった。なお、コホート1の1例は48週時点で8歳未満であったため、患者によるPGIC(痒み/引っ掻き)評価は行わなかった。 •24週時及び48週時のPGIC(睡眠)は、コホート1の1例では患者A little better及びNo change/保護者Moderately better及びA little better/医師Moderately better及びModerately better、他の1例では保護者Very much better及びModerately better/医師Very much better及びVery much betterであった。コホート2の1例では患者A little better及びA little worse/保護者A little better及びA little worse/医師No change及びNo changeであった。最終評価時点のPGIC(睡眠)は、コホート1の1例では患者A little better/保護者A little better/医師Moderately better(112週時)、コホート1の他の1例では保護者Very much better/医師Very much better(112週時)であった。コホート2の1例では患者No change/保護者No change/医師No change(96週時)であった。なお、コホート1の1例は48週時点で8歳未満であったため、患者によるPGIC(睡眠)評価は行わなかった。 •本治験期間中に肝移植、及び胆汁内瘻術又は胆汁外瘻術を受けた被験者はいなかった。 •コホート1の1例ではALTが経時的に低下し、ベースライン135 U/L、24週時32 U/L、48週時27 U/L、72週時15 U/L、及び84週時16 U/Lであった。 |
- One subject in Cohort 1 experienced a fasting s-A concentration decrease of 70% or more from baseline to Week 24, or reached a fasting s-BA concentration of 70 micro mol/L or less after 24 weeks of treatment, i.e. the primary endpoint, throughout the dose adjustment period. The reduction was observed at Week 22, Weeks 24 and Week 24 (average) for the subject, while the other subject in Cohort 1 and the subject in Cohort 2 did not reach the s-BA concentration decrease criteria at any timepoints of up to Week 24. - For visits up to Week 48, the s-BA concentration reached 70 micro mol/L or less at Weeks 24 to Week 48 (average) in one subject in Cohort 1, showing a significant decrease from baseline. The other subject in Cohort 1 did not have a baseline s-BA value available therefore a change from baseline could not be calculated, however none of their on treatment values reached a level < 70 micro mol/L and there was no significant decrease in values over time. In the subject in Cohort 2, the s-BA concentration decreased slightly over time to Week 48. No subjects met the criteria for ""subjects who experienced a fasting s-BA concentration decrease of 70% or more from baseline to Week 48. - From Week 48 onward, the s-BA concentration remained below 70 micro mol/L until the last assessment time point (Week 112) in one subject in Cohort 1, showing a significant decrease from baseline. The other subject in Cohort 1 did not have a baseline s-BA value available therefore a change from baseline could not be calculated, however none of their on treatment values reached a level < 70 micro mol/L and there was no significant decrease in values over time. In the subject in Cohort 2, the s-BA concentration decreased through the overall period from baseline (248.1 micro mol/L), and a decrease of 70% or more from baseline was observed at Week 94 (72.1 micro mol/L). - For visits up to Week 48 and from Week 48 onward, the proportion of subjects with positive pruritus assessments (ObsRO, AM and PM scores combined) in Cohort 1 was 50.0% (1 of 2 subjects) at Weeks 12 to 32, 40 to 48, and 60, 70, 72, 84, 94 and 96. In Cohort 2, it was 100% (1 of 1 subject) at Weeks 4, 8, 16 and 84. - One subject in Cohort 1, the height, weight were below the values of the standard growth curve at each assessment time point, while the BMI was slightly higher. In the other subject in Cohort 1, the height was below the value of the standard growth curve from baseline to Week 22 and at Week 28 but slightly exceeded it at most assessment time points. The weight and BMI exceeded the values of the standard growth curve at each assessment time point. The subject in Cohort 2, the height, weight, and BMI were below the values of the standard growth curve at each assessment time point. - For the PGIC (itching/scratching score) at Weeks 24 and 48, in one subject in Cohort 1, the patient reported ""A little better"" and ""No change"", the guardian reported ""Moderately better"" and ""A little better"", and the physician reported ""No change"" and ""No change"". For the other subject in Cohort 1, the guardian reported ""Very much better"" and ""Moderately better"", and the physician reported ""Very much better"" and ""Moderately better"". The subject in Cohort 2 reported ""No change"" and ""No change"", the guardian reported ""A little better"" and ""No change"", and the physician reported ""No change"" and ""A little better"". At last assessment time point, for the PGIC (itching/scratching score), one subject in Cohort 1 reported ""A little better"", the guardian reported ""Moderately better"", and the physician reported ""A little better"" (Week 112). For the other subject in Cohort 1, the guardian reported ""Very much better"", and the physician reported ""Very much better"" (Week 112). The subject in Cohort 2 reported ""No change"", the guardian reported ""A little better"", and the physician reported ""A little better"" (Week 96). Note that the other subject in Cohort 1 was under 8 years old at Weeks 24 and 48, so the PGIC assessment was not conducted by the subject. - For the PGIC (sleep score) at Weeks 24 and 48, One subject in Cohort 1 reported ""A little better"" and ""No change"", the guardian reported ""Moderately better"" and ""A little better"", and the physician reported ""Moderately better"" and ""Moderately better"". For the other subject in Cohort 1, the guardian reported ""Very much better"" and ""Moderately better"", and the physician reported ""Very much better"" and ""Very much better"". The subject in Cohort 2 reported ""A little better"" and ""A little worse"", the guardian reported ""A little better"" and ""A little worse"", and the physician reported ""No change"" and ""No change"". At last assessment time point, for the PGIC (sleep score), one subject in Cohort 1 reported ""A little better "", the guardian reported ""A little better"", and the physician reported ""Moderately better"" (Week 112). For the other subject in Cohort 1, the guardian reported ""Very much better"", and the physician reported ""Very much better"" (Week 112). The subject in Cohort 2 reported ""No change"", the guardian reported ""No change"", and the physician reported ""No change"" (Week 96). Note that the other subject in Cohort 1 was under 8 years old at Weeks 24 and 48, so the PGIC assessment was not conducted by the subject. - No subject underwent liver transplantation or biliary diversion surgery (endobiliary or extrabiliary fistula surgery) throughout the study period. - In one subject in Cohort 1, ALT decreased over time and was 135 U/L at baseline, 32 U/L at Week 24, 27 U/L at Week 48, 15 U/L at Week 72, and 16 U/L at Week 84. |

|
以下の基準をすべて満たす患者を本治験に組み入れる。 1. コホート 1: Visit 1(同意取得時)において生後6 ヵ月以上18 歳未満かつ体重が5 kg 超であり、遺伝子検査においてATP8B1 遺伝子又はABCB11 遺伝子で両対立遺伝子病原性変異が確認され、PFIC-1 又はPFIC-2 と診断されている日本人男女 2. コホート 2: 遺伝子検査を実施したが上記1 に該当しない、Visit 1(同意取得時)において生後3 ヵ月以上かつ体重が5 kg 超の日本人PFIC 患者。 ただし、コホート1 の組入れ期間終了後は、コホート1 に適格な被験者もコホート2 に組み入れる。 3. スクリーニング期間中、少なくとも7 日間の間隔を空けて採取した2 検体(Visit 1 と2)において、血清中胆汁酸濃度が高値(測定値が平均100 μmol/L 以上)の者 4. 重大なそう痒の既往歴を有し、保護者が記入した日誌で、Visit 3 前の2 週間の引っ掻き行動の平均スコアが2 以上(0~4 の尺度)と報告されている者。 ただし、コホート2 で被験者が18 歳以上の場合は、重大なそう痒の既往歴を有し、被験者が記入した日誌で、Visit 3 前の2 週間の痒みの平均スコアが2 以上(0~4 の尺度)と報告されている者 5. 患者及び/又は代諾者が同意説明文書(インフォームド・コンセント又は15 歳未満の場合はアセント)に署名ができる者。治験期間中に18 歳に達した患者では、試験を継続するために再度同意を取得する。 6. 治験薬投与を24 週間以上継続できると治験責任(分担)医師により判断された者 7. 治験期間を通じて保護者の支援が得られる見込みがある者(保護者は両親又は祖父母が望ましい。患者日誌等の質問票に回答する保護者は固定するのが望ましいが、やむを得ない場合は両親又は祖父母であれば変更可能とする) なお、コホート2 については被験者がVisit 1(同意取得時)において18 歳以上の場合、本治験での保護者の設定を必須としない。 8. 保護者及び規定の年齢に達している患者(8 歳以上)並びに本治験中に8 歳に達した患者は、治験で求められる患者日誌を使用する意思があり、使用できる者 |
Patients will be enrolled in the study if they meet all of the following criteria: 1. (Cohort 1) A Japense male or female patient, with diagnosis of PFIC Type 1 or 2 through identification of biallelic pathogenic variants in either the ATP8B1 or ABCB11 genes, between the ages of >=6 months and <18 years at Visit 1 (informed consent) with a body weight above 5 kg. 2. (Cohort 2) Among patients who do not meet item 1 above upon confirmation of gene examination, those with PFIC aged 3 months or older at Visit 1 (informed consent) and weighed above 5 kg. After the enrollment period of Cohort 1, patients who are eligible for Cohort 1 will be included in Cohort 2. 3. During the screening period, patient must have elevated s-BA concentration, specifically measured to be >=100 micro mol/L, taken as the average of 2 samples at least 7 days apart (Visits 1 and 2) prior to randomization 4. Patient must have history of significant pruritus and a patient or guardian-reported observed scratching in the Patient Diary average of >=2 (on 0 to 4 scale) in the 2 weeks prior to Visit 3. However, if the patient in Cohort 2 is 18 years or older, the patient must have a history of significant pruritus and a patient -reported observed scratching in the Patient Diary with average itching score of >=2 (on 0 to 4 scale) in the 2 weeks prior to Visit 3. 5. Patient and/or legal guardian who can sign informed consent (or assent if the patient is below 15 years old) as appropriate. Patients who turn 18 years of age (or legal age per country) during the study will be required to re-consent in order to remain in the study 6. Patients who are judged by the investigator or subinvestigator to be able to continue treatment with the study drug for at least 24 weeks. 7. Patients will be expected to have a consistent guardian for the duration of the study (Parents and grandparents are preferred as guardians. It is preferable that the same guardian will answer the patient diary and other questionnaires, but parents and grandparents can be changed if it is unavoidable). For Cohort 2, if the patient is 18 years of age or older at the time of Visit 1 (when informed consent is obtained), setting of the guardian in this study is not mandatory. 8. Guardians and age-appropriate patients (>=8 years of age) and patients who achieve 8 years of age during the study must be willing and able to use an Patient Diary as required by the study |

以下の基準のいずれかに該当する患者は、本治験から除外する。 1. 胆汁酸塩排出ポンプ(BSEP)蛋白の完全な欠如が予測されるABCB11 遺伝子の病理学的変異を有する者 2. 原疾患以外の種類の肝疾患の既往歴又は合併症を有する者(以下の疾患を含むが、これらに限定されな い): a) 胆道閉鎖症(種類を問わない) b) 正常な血清中胆汁酸の検査結果によって示される良性の再発性肝内胆汁うっ滞 c) 画像検査で明らかとなった肝臓がん若しくは肝臓への転移、又はその疑い d) 肝生検の組織病理学的検査で示唆された胆汁うっ滞の別の非PFIC 関連病因 3. 腸内の薬物の吸収、分布、代謝(特に胆汁酸代謝)又は排泄を妨げることが知られているその他の疾患 又は状態の既往歴又は合併症(炎症性腸疾患を含むがこれに限定されない)を有する者 4. 静脈内輸液又は栄養療法を必要とする慢性(3 ヵ月超持続)の下痢の既往歴又は合併症を有する者 5. 以下のいずれかを有する者: a) ヒト免疫不全ウイルス感染の診断歴 b) 上記以外で現在発症中の活動性で臨床的に重要な急性又は慢性の感染症 c) 治験薬の投与開始(Day 1)の4 週間以内に入院を要した主要な感染症エピソードの既往歴 d) 治験薬の投与開始(Day 1)の4 週間以内の非経口抗感染治療 e) スクリーニング期間の開始前2 週間以内に経口抗感染治療が完了 6. 同意取得前 5 年以内に悪性腫瘍の診断及び治療歴のある患者(皮膚の基底細胞癌の患者は除く) 7. 腎機能障害を伴う慢性腎臓病の既往歴を有し、かつスクリーニング時の糸球体濾過量が70 mL/min/1.73 m2 未満の者 8. スクリーニング期間の開始前6 ヵ月以内に胆汁内瘻術又は胆汁外瘻術の手術歴を有する者 9. 過去に肝移植を受けた者又は同意取得後6 ヵ月以内に肝移植が予定されている者 10. 非代償性肝疾患、凝固障害、臨床的に重要な腹水の既往歴又は合併症、静脈瘤出血、及び/又は脳症を有する者 11. スクリーニング時の国際標準化比(INR)が1.4 超の者 (ビタミン K の静脈内投与で治療され、再度の検体採取時にINR が1.4 以下の場合、患者は組み入れられる可能性がある) 12. スクリーニング時の血清ALT 値が基準値上限(ULN)の10 倍超の者 13. 過去 6 ヵ月間の任意の時点で血清ALT 値が15×ULN 超であり、その上昇に関するPFIC 以外の原因が確認されていない者 14. スクリーニング時の総ビリルビン値がULN の10 倍超の者 15. PFIC 以外のコントロール不能で治療抵抗性のそう痒状態を有する者(例として、難治性アトピー性皮膚炎又はその他の原発性そう痒性皮膚疾患を含むが、これらに限定されない) 16. 妊娠中若しくは授乳中の者、又は妊娠を希望する者 17. 性的に活発な男性及び女性の場合、治験期間を通じて及びその後90 日間(同意説明文書への署名時から治験薬の最終投与後90 日まで)に失敗率が1%以下の信頼できる避妊法(ホルモン避妊、子宮内避妊 具、又は完全な禁欲など)を使用しない者 18. 過去にアルコール又は薬物乱用の既往歴を有する者患者は治験期間中、違法薬物及びアルコールの使用を控えることに同意する必要がある。 19. Visit 1(同意取得時)で胆汁酸又は脂質結合樹脂、消化管機能に影響を与える薬物が投与されている者(Appendix 1 併用薬使用ガイドラインを参照) 20. 同意取得前 30 日以内に治験薬を用いる臨床試験に参加した者 21. Visit 1(同意取得時)前30 日以内又はその他の治験薬の半減期の5 倍のいずれか長い方の期間中に、薬物、生物学的製剤又は医療機器に治験的曝露された者 22. 過去に胆汁酸トランスポーター(IBAT)阻害剤で治療されたが、痒みに効果がなかったと担当医が判断した者。 なお、同意取得時点でIBAT 阻害剤を服用している場合、同意取得日以降、IBAT 阻害剤を中止する必要がある。 23. フェニル酪酸ナトリウムを服用している場合、スクリーニング期間の開始前6 ヵ月以内に用法・用量を変更した者(体重変動による用法・用量を変更を除く) 24. 治験責任(分担)医師若しくは治験依頼者により、患者の安全を損なう可能性のある、又は患者の本治験への参加若しくは完了を妨げる可能性のあるその他の状態若しくは異常を有すると判断された者 25. 治験実施医療機関又は治験依頼者の職員及びその家族 26. 治験期間中に手術の予定がある者 |
Patients who meet any of the following criteria will be excluded from the study: 1. Patient with pathologic variations of the ABCB11 gene that predict complete absence of the BSEP protein 2. Patient with past medical history or ongoing complication of liver disaese other than underlying disease, but not limited to, the following: a) Biliary atresia of any kind b) Benign recurrent intrahepatic cholestasis, indicated by any test results of normal s-Bas c) Suspected or proven liver cancer or metastasis to the liver on imaging studies d) Histopathology on liver biopsy is suggestive of alternate non-PFIC related etiology of cholestasis 3. Patient with a past medical history or ongoing presence of any other disease or condition known to interfere with the absorption, distribution, metabolism (specifically bile acid metabolism), or excretion of drugs in the intestine, including but not limited to, inflammatory bowel disease 4. Patient with past medical history or ongoing chronic (i.e., >3 months) diarrhea requiring intravenous fluid or nutritional intervention for treatment of the diarrhea and/or its sequelae 5. Patient who has the followings: a) A confirmed past diagnosis of infection with human immunodeficiency virus b) Present and active, clinically significant, acute, or chronic infection c) Past medical history of any major episode of infection requiring hospitalization within 4 weeks of treatment start (study Day 1) d) Treatment with parenteral anti-infective treatment within 4 weeks of treatment start (study Day 1) e) Completion of oral anti-infective treatment within 2 weeks prior to start of Screening Period 6. Patient who have diagnosis and treatment history of malignancy within 5 years prior to obtaining concent. 7. Patient with a past medical history of chronic kidney disease with an impaired renal function and a glomerular filtration rate <70 mL/min/1.73 m2 at Screening. 8. Patient with surgical history of endobiliary or extrabiliary fistula within 6 months prior to start of Screening Period. 9. Patient who has a history of liver transplantation or plan to undergo liver transplantation within 6 months after obtaining consent 10. Decompensated liver disease, coagulopathy, history or presence of clinically significant ascites, variceal hemorrhage, and/or encephalopathy 11. International normalized ratio (INR) >1.4 at Screening (the patient who is treated with Vitamin K intravenously, and the INR is =<1.4 at resampling may be enrolled) 12. Serum ALT >10^ upper limit of normal (ULN) at Screening 13. Serum ALT >15^ ULN at any time point during the last 6 months and the alternate etiology for the elevation is not confirmed 14. Total bilirubin >10^ ULN at Screening 15. Patient suffers from uncontrolled, recalcitrant pruritic condition other than PFIC. Examples include, but not limited to, refractory atopic dermatitis or other primary pruritic skin diseases 16. Any patient who is pregnant or lactating or who is planning to become pregnan 17. Sexually active males and females who are not using a reliable contraceptive method with =<1% failure rate (such as hormonal contraception, intrauterine device, or complete abstinence) throughout the duration of the study and 90 days thereafter (from signed informed consent through 90 days after last dose of study drug). 18. Patient with a past medical history of alcohol or substance abuse will be excluded. Patient must agree to refrain from illicit drug and alcohol use during the study 19. Administration of bile acid or lipid binding resins and medications that affect GI function (Refer to Appendix 1 - Concomitant Medications Guidelines) at Visit 1 (obtaining concent) 20. Patients who have participated in a clinical trial using the investigational drug within 30 days prior to obtaining consent 21. Patient has had investigational exposure to a drug, biologic agent, or medical device within 30 days prior to Visit 1 (obtaining concent), or 5 half-lives of the other study agent, whichever is longer 22. Patient who has been previously treated with an IBAT inhibitor whose pruritus has not responded to treatment Of note, if the patient is taking IBAT inhibitors at the time of informed consent, IBAT inhibitors must be discontinued from the date of informed consent. 23. Patient who have changed dosage within 6 months prior to screening if taking sodium phenylbutyrate (excluding patients who have changed dosage due to weitht change) 24. Any other conditions or abnormalities which, in the opinion of the investigator or subinvestigator or the sponsor, may compromise the safety of the patient, or interfere with the patient participating in or completing the study 25. Personnel and family members of the study site or sponsor 26. Patients who are scheduled to undergo surgery during the study period |

通常、1日1回朝食時にA4250 (odevixibat)40 μg/kgを経口投与する。症状の改善がない場合は、1日120 μg/kg まで 増量可能。 | Subjects who met the eligibility criteria at the screening test will receive A4250 40 micro g/kg (once daily) of study drug from the next day of Visit 3 (Day 1) for 12 weeks. If no safety problems are observed after 12 weeks (Visit 6), the dose of study drug will be increased to 120 micro g/kg, in principal, and administered for 12 weeks (the study drug will be administered for a total of 24 weeks). After Week 12 (Visit 6), the investigator will select the dose of study drug (either 40 micro g/kg or 120 micro g/kg) according to the subjects safety and tolerability. | |
