/ 合計81名の参加者が試験に登録された(日本からの参加者6名を含む);40名の参加者はプラセボ群に、41名の参加者はボソリチド群に無作為に割り付けられた。52週間の治療後、プラセボ群の参加者39名とボソリチド群の参加者41名全員が試験を完了した。
プラセボ群の参加者の一人は、注射の継続を望まなくなったため7日目に試験治療を中止し、その後56日目に試験を中止した (参加者による中止) 。
A total of 81 participants were enrolled into the study (including 6 participants from Japan); 40 participants were randomized to receive placebo and 41 participants to receive vosoritide. After 52 weeks of treatment, 39 participants in the placebo group and all 41 participants in the vosoritide group completed the study.
One participant in the placebo group withdrew from study treatment on Day 7 as the participant no longer wanted to continue taking injections and later withdrew from the study on Day 56 (withdrawal by participant).
/ ボソリチド群における曝露期間は41.05人・年であり、HCH患者に対するボソリチドの連日投与の忍容性は良好であった。
両投与群の参加者の大多数が試験期間中に少なくとも1回の有害事象を経験し(プラセボ群72.5%、ボソリチド群87.8%)、曝露量で補正した有害事象発生率はそれぞれ2.71及び3.85 AE/PYであった。
試験期間中に死亡又はCTCAE Grade 3以上の有害事象は報告されなかった。プラセボ群の参加者1名に発作のSAEが報告された。ボソリチド群ではSAEは報告されなかった。
有害事象はほとんどが非重篤で、重症度はGrade 1又は2であり、投与又は試験中止には至らなかった。
投与に関連する有害事象は、プラセボ群の参加者2名(5.0%)及びボソリチド群の参加者10名(24.4%)に報告された。これらはいずれもSAE又は重症度Grade 3以上ではなかった。
プラセボ群の参加者1名及びボソリチド群の参加者2名に、重症度がGrade 2以上又は24時間を超えて持続した(皮下出血を除く)注射部位反応(ISR)が報告された。ISRにより試験又は試験治療を中止した参加者はいなかった。
血圧低下に関連する事象はまれであり、重症度は軽度であった。ボソリチド群では、2名(4.9%)に低血圧が報告され、うちPT:血圧低下(無症候性であるが臨床的に有意と評価)及びPT:低血圧(Grade 1)が報告された。いずれも投与直後に発現し、支持療法(安静及び経口補液)で回復し、治療の変更は必要なかった。専用のeCRFには、3名の参加者の症状に基づく事象が追加記録されており、1名は投与後の測定された血圧低下、1名は失神/処置後の浮動性めまいのエピソード、1名は血圧との関連が不明な「前失神」(Pre-syncope)の事象であり、分類はデータベースロック時にレビュー中であった。
プラセボ群では頻脈(Grade 1)が1件報告された。治験薬の休薬につながる心血管イベントはなく、全て回復した。
過敏症の有害事象はプラセボ群で7名(17.5%)、ボソリチド群で9名(22.0%)に報告された。重篤な有害事象はなく、休薬又は中止に至った有害事象はなかった。
ボソリチド群の1名は、スポーツ関連の損傷によるGrade 2の橈骨骨折を経験したが、治験薬との関連はないと評価された。
プラセボ群の1名(2.5%)は、発作のGrade 2のSAEを経験し、治験責任医師の報告によると、その後両側側頭葉基底部の脳回形成異常と診断された。HCHでは、このような側頭葉形成不全が報告されている。
耳炎関連の有害事象はプラセボ群3名(7.5%)、ボソリチド群3名(7.3%)に認められた。中耳炎はHCHの小児に特に多くみられ、本試験では専用のeCRFを介して特にモニタリングされた。
治験実施計画書には、さらなるモニタリングの必要性について治験責任医師を導くための血圧ガイドラインが記載されていた。平均収縮期血圧(SBP)には、ボソリチド群及びプラセボ群のいずれにおいても顕著な変化は認められなかった。ボソリチド群における拡張期血圧(DBP)及び心拍数(HR)の平均変化は最小限であった。投与後のSBP低下は、プラセボ群の3名(7.5%)及びボソリチド群の8名(19.5%)で70 mmHg+年齢の2倍未満であったが、いずれの投与群においても投与前40 mmHg以上であったDBPが投与後40 mmHg未満に低下した参加者はいなかった。所定の血圧閾値を満たす血圧変化はいずれも無症候性であり、追加のモニタリングが必要となったが、治験責任医師(PI)の判断における有害事象の定義には該当しなかった。
試験期間中、アナフィラキシー、大腿骨頭すべり症(SCFE)、無血管壊死/骨壊死の事象は報告されなかった。
Daily administration of vosoritide was well tolerated in participants with HCH with 41.05 person-years of exposure in the vosoritide group.
The majority of participants in both treatment groups experienced at least 1 AE during the study (72.5% in the placebo group and 87.8% in the vosoritide group) with exposure-adjusted event rates of 2.71 and 3.85 AEs/PY, respectively.
No deaths or AEs of CTCAE Grade >=3 were reported during the study. One SAE of seizure was reported in one participant in the placebo group. No SAEs were reported in the vosoritide group.
Adverse events were mostly non-serious, Grade 1 or 2 in severity and did not lead to treatment or study discontinuation.
Treatment-related AEs were reported in 2 (5.0%) participants in the placebo group and 10 (24.4%) participants in the vosoritide group; none of these were SAEs or Grade 3 or higher in severity.
One participant in the placebo group and 2 participants in the vosoritide group reported ISRs that were either Grade >=2 in severity or lasted >24 hours (excluding bruising); no participant discontinued the study or study treatment due to ISRs.
Events related to blood pressure decreases were infrequent and mild in severity. In the vosoritide group, 2 (4.9%) participants reported events of hypotension including one event of PT: decreased blood pressure (asymptomatic but assessed as clinically significant) and one event of PT: hypotension (Grade 1), which occurred shortly after dosing, resolved with supportive measures (rest and oral hydration), and did not require treatment modification. A dedicated eCRF additionally recorded symptom-based events in 3 participants: one with a measured post-dose blood pressure decrease, one with an episode of syncope/post-procedural dizziness and one with a "lipothymic" (pre-syncopal) event with uncertain relationship to blood pressure, the classification of which was under review at database lock.
One event of tachycardia (Grade 1) was reported in the placebo group. No cardiovascular events led to study drug interruption, and all resolved.
Hypersensitivity AEs were reported in 7 (17.5%) participants in the placebo group and 9 (22.0%) participants in the vosoritide group; none were serious and none led to treatment interruption or discontinuation.
One participant in the vosoritide group experienced a Grade 2 radius fracture due to a sports-related injury that was assessed as not related to study treatment.
One (2.5%) participant in the placebo group experienced a Grade 2 SAE of seizure and was subsequently diagnosed with bilateral temporobasal dysgyria, as reported by the investigator. Such temporal lobe dysgenesis has been reported in HCH.
Otitis adverse events were reported in 3 (7.5%) participants in the placebo group and 3 (7.3%) participants in the vosoritide group. Otitis media events are particularly common in children with HCH and were monitored specifically via a dedicated eCRF in this study.
Blood pressure guidelines were provided in the protocol for investigators to guide investigators regarding the need for further monitoring. No notable changes were seen in mean systolic blood pressure (SBP) in either the vosoritide or placebo group. The mean changes in DBP and HR in the vosoritide group were minimal. Postdose decreases in SBP <70 mmHg plus 2x age were reported in 3 (7.5%) participants in the placebo group and 8 (19.5%) participants in the vosoritide group, while no participant in either treatment group reported decreases in DBP from a >= 40 mmHg pre-dose value to post-dose DBP of <40 mmHg. All changes in blood pressure meeting the predefined blood pressure thresholds were asymptomatic, prompted additional monitoring, but did not meet the definition of an AE in the judgement of the PI.
No events of anaphylaxis, SCFE, or avascular necrosis/osteonecrosis were reported during the study.

ベースラインから Week 52時までの身長の最小二乗平均変化量は、プラセボ群で4.32 cm(95%信頼区間:3.98, 4.67)、ボソリチド群で6.68 cm(95%信頼区間:6.34, 7.01)であった。ベースラインからの平均身長変化量の差は2.35 cm(95%信頼区間:1.86, 2.85;片側p<0.0001)であり、ボソリチド群に有利な統計学的有意差が認められた。

ベースラインからWeek 52時までの身長Zスコアの最小二乗平均変化量は、プラセボ群で–0.05 SDS(95%信頼区間:–0.12, 0.03)、ボソリチド群で+0.34 SDS(95%信頼区間:0.27, 0.42)であった。ベースラインからの最小二乗平均変化量の差は0.39 SDS(95%信頼区間:0.28, 0.50;片側p<0.0001)であり、ボソリチド群に有利な統計学的有意差が認められた。

The least-squares (LS) mean change from baseline to Week 52 in AGV was -0.39 cm/year (95% confidence interval [CI]: -0.73, -0.04) in the placebo group and 1.95 cm/year (95% CI: 1.61, 2.28) in the vosoritide group. The difference in LS mean change from baseline was 2.33 cm/year (95% CI: 1.85, 2.82) in favor of vosoritide. Superiority of vosoritide over placebo was established, as evidenced by the one-sided p-value < 0.0001.

Standing Height
The LS mean change from baseline to Week 52 in standing height was 4.32 cm (95% CI: 3.98, 4.67) in the placebo group and 6.68 cm (95% CI: 6.34, 7.01) in the vosoritide group. The difference in LS mean change from baseline was 2.35 cm (95% CI: 1.86, 2.85; one-sided p < 0.0001), indicating statistically significant results favoring vosoritide.

Height Z-Score Referenced to Average Stature (CDC 2000)
The LS mean change from baseline to Week 52 in height Z-score was -0.05 SDS (95% CI: -0.12, 0.03) in the placebo group and +0.34 SDS (95% CI: 0.27, 0.42) in the vosoritide group. The difference in LS mean change from baseline was 0.39 SDS (95% CI: 0.28, 0.50; one-sided p < 0.0001), indicating statistically significant results favoring vosoritide.
/ ボソリチドを1日1回体重幅別の投与法で投与すると、 年間成長速度(AGV) が改善し(治療差2.33 cm/年;p<0.0001)、副次的評価項目である身長、身長Zスコア、腕の長さに一貫した改善が認められた。ボソリチドは忍容性が高く、安全性プロファイルは軟骨無形成症(ACH)における臨床試験のデータと一致していた。 Vosoritide administered once daily via weight-band dosing regimen produced an improvement in AGV (treatment difference 2.33 cm/year; p<0.0001), with consistent gains across secondary endpoints of standing height, height Z-score, and arm span. Vosoritide was well tolerated, with a safety profile consistent with data from ACH clinical studies.

1. 組入れ時に3歳以上18歳未満の患者
2. 遺伝子検査によりHCHの遺伝子診断が確定している.
3. 無作為化前に6ヵ月間以上の立位身長の投与前評価がある。
4. 米国疾病予防管理センター(CDC)の成長曲線を用いて算出した身長Zスコアが同性・同年齢の一般集団と比較して−2.0 SDS以下である。
5. 標準治療に従ってGreulich-Pyle法(Greulich 1971)により左手の前後(AP)X線写真を用いて骨端線閉鎖前の状態であると判定されている。
6. 男性及び女性が本治験に参加可能である。
7. 10歳以上又は初経後の女性は、スクリーニング来院時の妊娠検査が陰性であり、治験期間中に追加の妊娠検査を受ける意思がある。
8. 性的に活動的である場合、治験参加中、効果の高い避妊法を使用する意思がある。
1. Participants must be >= 3 to < 18 years of age at enrollment
2. A confirmed genetic diagnosis of HCH
3. Have at least a 6-month period of pre-treatment standing height assessments prior to randomization.
4. A height Z score of <= - 2.0 standard deviations (SDs) in reference to the general population of the same age and sex, as calculated using the Center for Disease Control and Prevention (CDC) growth charts
5. Open epiphyses as assessed by left hand antero-posterior (AP) X-rays, by the Greulich and Pyle method (Greulich 1971), as per standard of care.
6. Males and females are eligible to participate in this clinical study.
7. Females >= 10 years old or who have begun menses must have a negative pregnancy test at the Screening Visit and be willing to have additional pregnancy tests during the study.
8. If sexually active, participants must be willing to use a highly effective method of contraception while participating in the study.

2. 本治験中に外科的介入を必要とする可能性が高い不安定な疾患を有する。
3. 成長速度の低下及び/又は成長板閉鎖のエビデンスがある。
4. 併用禁止薬のいずれかを使用している。
5. 成長ホルモン、インスリン様成長因子1(IGF-1)又は蛋白同化ステロイドによる治療を受けた。
6. 治験期間中に四肢延長術が計画されている、又は予想される。
7. 治験期間中に骨関連手術(骨皮質の破壊を伴う手術、抜歯を除く)が計画されている、又は予想される。
8. 治験期間完了前に他の治験薬を必要とする。
9. 過去にボソリチド若しくは他の治験薬、又は治験用医療機器を使用した。
10. いずれかの時点で、HCH又は低身長の治療として治験薬(ボソリチドを含む)又は治験用医療機器を使用したことがある。
11. 悪性腫瘍に罹患している、悪性腫瘍の既往歴がある、又は悪性腫瘍の疑いについて現在精密検査を受けている。
12. ボソリチド又はその添加剤に対する既知の過敏症
13. 何らかの理由により治験参加又は安全性評価を妨げると治験担当医師が判断する疾患又は状態が併存する。
1. Short stature condition other than HCH
2. Have an unstable condition likely to require surgical intervention during the study.
3. Evidence of decreased growth velocity and/or growth plate closure
4. Taking any of the prohibited medications
5. Treated with growth hormone, insulin-like growth factor 1 (IGF-1), or anabolic steroids
6. Planned or expected to have limb-lengthening surgery during the study period.
7. Planned or expected bone-related surgery (ie, surgery involving disruption of bone cortex, excluding tooth extraction), during the study period
8. Require any investigational agent prior to completion of study period.
9. Received vosoritide or another investigational product or investigational medical device in the past
10. Have used any investigational product or investigational medical device for the treatment of HCH or short stature at any time, including vosoritide
11. Have current malignancy, history of malignancy, or currently under work-up for suspected malignancy.
12. Have known hypersensitivity to vosoritide or its excipients.
13. Concurrent disease or condition that, in the view of the investigator, would interfere with study participation or safety evaluations, for any reason.
