1. 本治験の手順を開始する前に、Main同意説明文書(informed consent form: ICF)に署名及び同意日を記入した。 2. Main ICFへの署名時点で18歳以上の成人である(治験参加のための法定同意年齢が18歳超の場合、各国の規制要件に従う)。 3. 外科的切除又は根治的化学放射線療法の候補とならないステージIIIbもしくはIIIC、又はステージIVの非扁平上皮非小細胞肺癌(non-squamous non-small cell lung cancer: NSQ NSCLC)が組織学的に確認されている。 4. 治験薬初回投与前28日以内の心エコー(echocardiogram: ECHO)又はマルチゲート心プールシンチグラフィ(multigated acquisition scan: MUGA)で左室駆出率(left ventricular ejection fraction: LVEF)が50%以上である。 5. 治験薬投与開始前14日以内に評価された米国東海岸癌臨床試験グループ(Eastern Cooperative Oncology Group: ECOG)パフォーマンスステータス(performance status: PS)が0又は1である。 6. 充分な臓器機能が保たれていること。 7. 固形癌の効果判定基準第1.1版(Response Evaluation Criteria in Solid Tumors version 1.1: RECIST v1.1)に基づく治験責任医師の判定による測定可能病変を有する。 サブスタディ1における追加の選択基準: 1. 局所進行切除不能又は転移性NSCLCに対して全身療法による前治療歴がない。 サブスタディ2における追加の選択基準: 1. Actionable遺伝子変異(actionable genetic alterations: AGA) (EGFR遺伝子変異を除く)を有する参加者:進行癌に対するAGAを標的とする治療と進行癌に対するプラチナ製剤による化学療法の治療歴がある。 2. AGAを有さない参加者:進行癌に対するプラチナ製剤による化学療法、及び抗programmed cell death protein 1/programmed death-ligand 1(PD-1/PD-L1)抗体の治療歴がある。 3. Part 2のみ:進行癌に対して1次又は2次治療の治療歴がある。 |
1. Sign and date the Main Trial informed consent form (ICF), prior to the start of any trial-specific procedures. 2. Adults greater than or equal to (>=) 18 years of age at the time the Main Trial ICF is signed (follow local regulatory requirements if the legal age of consent for trial participation is >18 years old). 3. Histologically documented Stage IIIB, IIIC disease who is not candidate for surgical resection or definitive chemoradiation, or Stage IV NSQ NSCLC. 4. Has a left ventricular ejection fraction (LVEF) >=50 percent (%) by either an echocardiogram (ECHO) or multigated acquisition scan (MUGA) within 28 days of the first trial intervention. 5. Eastern Cooperative Oncology Group (ECOG) performance status (PS) of 0 or 1 assessed no more than 14 days prior to initiation of trial interventions. 6. Has adequate organ function. 7. Participants must have measurable disease by investigator assessment according to RECIST v1.1. Additional Inclusion Criteria for Substudy 1: 1. Participants must not have received prior systemic therapy for locally advanced unresectable or metastatic NSCLC. Additional Inclusion Criteria for Substudy 2: 1. Participants with AGA (excluding EGFR mutation): Participants have been previously treated with targeted therapy for the AGA and platinum-based chemotherapy for the advanced disease setting. 2. Participants without AGA: Participants have been previously treated with platinum-based chemotherapy and anti-programmed death-1 (anti-PD-1)/programmed death-ligand 1 (PD-L1) antibody. 3. Part 2 only: Participants must have received only 1 or 2 prior lines of anticancer therapy for the advanced disease setting. |

1. ムチン1(mucin 1: MUC1)又はTA-MUC1を標的とする全身性の抗がん治療歴がある。 2. トポイソメラーゼ1阻害薬又はトポイソメラーゼ1阻害薬を有する抗体薬物複合体(antibody-drug conjugate: ADC)(例えば、ダトポタマブ デルクステカン及びサシツズマブ ゴビテカン)による全身性の抗がん治療歴がある。 3. 脊髄圧迫又は臨床的活動性を示す中枢神経系(central nervous system: CNS)転移を有する。 4. 複数の原発性悪性腫瘍を有する。 5. ステロイド使用の有無を問わず、間質性肺疾患(interstitial lung disease: ILD)/肺臓炎の既往を有する、又は現在ILDを有する、もしくはILDが疑われる、あるいはスクリーニング時の画像検査によりILDを否定できない。 6. 活動性のヒト免疫不全ウイルス(human immunodeficiency virus: HIV)感染又はコントロール不良のHIV感染が認められる。 7. 活動性又はコントロール不良のB型肝炎ウイルス(hepatitis B virus: HBV)/C型肝炎ウイルス(hepatitis C virus: HCV)感染が認められる。 8. 過去6ヵ月以内に次のいずれかが認められている:脳血管発作、一過性脳虚血発作、又はその他の動脈血栓塞栓症。 9. 他の開発段階の治療的処置を現在受けている(ただし、開発段階の治療を伴わない長期観察への参加は除く)。 |
1. Prior systemic anticancer therapy targeting MUC1 or TA-MUC1. 2. Prior systemic anticancer therapy with topoisomerase 1 inhibitor or topoisomerase 1 inhibitor-based antibody-drug conjugate (ADCs) (e.g., datopotamab deruxtexan and Sacituzumab govitecan). 3. Has spinal cord compression or clinically active central nervous system (CNS) metastases. 4. Has multiple primary malignancies. 5. Has any history of interstitial lung disease (ILD)/pneumonitis irrespective of steroid use, or current ILD, or suspected ILD, or ILD that cannot be ruled out by imaging at screening. 6. Has active or uncontrolled human immunodeficiency virus (HIV) infection. 7. Has active or uncontrolled hepatitis B virus (HBV)/hepatitis C virus (HCV) infection. 8. Any of the following within the past 6 months: cerebrovascular accident, transient ischemic attack, or other arterial thromboembolic event. 9. Current participation in other therapeutic investigational procedures, except for participation in long term survival follow-up (LTSFU) without any investigational treatment. |

Experimental: Substudy 1: Part 1: Cohort A: DS-3939a + Pembrolizumab Participants will receive intravenous (IV) infusion of DS-3939a, every 3 weeks (Q3W) on Day 1 of each 21-day cycle until the time radiographic disease progression has been documented as assessed per Response Evaluation Criteria in Solid Tumors version 1.1 (RECIST v1.1) by investigator or other reason for treatment discontinuation has been met along with pembrolizumab, 200 milligrams (mg), Q3W on Day 1 of each 21-day cycle up to a maximum of 35 cycles or until treatment discontinuation criteria are me. Drug: DS-3939a, Pembrolizumab Experimental: Substudy 1: Part 1: Cohort B: DS-3939a + Pembrolizumab + Carboplatin Participants will receive IV infusion of DS-3939a, Q3W on Day 1 of each 21-day cycle until the time radiographic disease progression has been documented as assessed per RECIST v1.1 by investigator or other reason for treatment discontinuation has been met along with pembrolizumab, 200 mg, Q3W on Day 1 of each 21-day cycle up to a maximum of 35 cycles and carboplatin at area under the curve (AUC) 5, Q3W on Day 1 of each 21-day cycle up to maximum of 4 cycles or until the discontinuation criteria are met. Drug: DS-3939a, Pembrolizumab, Carboplatin Experimental: Substudy 1: Part 1: Cohort C: DS-3939a + Pembrolizumab + Pemetrexed Participants will receive IV infusion of DS-3939a, Q3W on Day 1 of each 21-day cycle until the time radiographic disease progression has been documented as assessed per RECIST v1.1 by investigator or other reason for treatment discontinuation has been met along with pembrolizumab, 200 mg, Q3W on Day 1 of each 21-day cycle up to a maximum of 35 cycles and pemetrexed 500 milligrams per meter square (mg/m^2), Q3W on Day 1 of each 21-day cycle until the discontinuation criteria are met. Drug: DS-3939a, Pembrolizumab, Pemetrexed Experimental: Substudy 1: Part 2: Cohort A: DS-3939a + Pembrolizumab Participants will receive IV infusion of DS-3939a at the recommended dose for expansion (RDE) based on Part 1 data, along with pembrolizumab, 200 mg, Q3W on Day 1 of each 21-day cycle up to a maximum of 35 cycles or until the discontinuation criteria are met. 1. パート1:用量制限毒性(DLTs)が認められた参加者数 [評価期間: 最初のサイクル期間(サイクルは21日間)] DLTsとは、原疾患又は原疾患に関連する経過、環境要因、治験薬と関連のない外傷などに起因せず、DLT評価期間(Day 1からサイクル1の終了まで)中に発現し、グレード3以上の治験治療下で発現した有害事象 treatment-emergent adverse event:TEAE)と定義する。毒性のグレードは米国国立がん研究所の有害事象共通用語規準(National Cancer Institute Common Terminology Criteria for Adverse Events:NCI-CTCAE) v6.0に従って判定する。 2. パート1:TEAEsが認められた参加者数 [評価期間: 最大約4年間] 有害事象(adverse event:AE)とは、医薬品が投与された参加者に生じたあらゆる好ましくない医療上のできごとであり、必ずしも当該医薬品の投与との因果関係が明らかなもののみを示すものではない。したがって、AEとは、医薬品が投与された際に起こる、あらゆる好ましくない、あるいは意図しない徴候(臨床検査値の異常を含む)、症状又は疾患のことであり、当該医薬品との因果関係の有無は問わない。 TEAEは、発現日又は増悪日が治療期間中(該当するサブスタディの治験実施計画書に別途規定されていない限り、治験薬の初回投与日から治験薬の最終投与の50日後まで)の有害事象と定義する。 3. パート2:RECIST v1.1に基づく治験責任医師の判定による客観的奏効(objective response:OR) [評価期間:最大約4年間] ORは、RECIST v1.1に基づく治験責任医師の判定により、最良総合効果(best overall response:BOR)が確定完全奏効(complete response:CR)又は確定部分奏効(partial response:PR)であった参加者と定義する。 |
Part 1: Number of Participants With Dose-limiting Toxicities (DLTs) [Time Frame: During first cycle (Cycle length=21 days)] DLT is defined as any treatment-emergent adverse event (TEAE) not attributable to disease or disease-related processes, environmental factors, unrelated trauma, etc., that occurs during the DLT-evaluation Period (Day 1 to the end of Cycle 1) and is Grade >=3. Toxicities will be graded according to the National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE) version 6.0. Part 1: Number of Participants With TEAEs [Time Frame: Up to approximately 4 years] An adverse event (AE) is any untoward medical occurrence in a participant administered a pharmaceutical product and that does not necessarily have to have a causal relationship with this treatment. An AE can therefore be any unfavorable and unintended sign (including an abnormal laboratory finding, for example), symptoms, or disease temporally associated with the use of a medicinal product, whether or not considered related to the medicinal product. TEAEs are defined as AEs with start or worsening date during the on-treatment period (from the first dose date of trial intervention to 50 days after the last dose date of trial intervention, unless otherwise specified in the applicable sub-study protocol). Part 2: Objective Response (OR) Per RECIST v1.1 as Assessed by Investigator [Time Frame: Up to approximately 4 years] OR is defined as participants with a best overall response (BOR) of confirmed response (CR) or confirmed partial response (PR) as assessed by investigator per RECIST v1.1. |

Part 1: OR Per RECIST v1.1 as Assessed by Investigator [Time Frame: Up to approximately 5 years] OR is defined as participants with a BOR of confirmed CR or confirmed PR as assessed by investigator per RECIST v1.1. 2. Part 2: Number of Participants With TEAEs [Time Frame: Up to approximately 5 years] An AE is any untoward medical occurrence in a participant administered a pharmaceutical product and that does not necessarily have to have a causal relationship with this treatment. An AE can therefore be any unfavorable and unintended sign (including an abnormal laboratory finding, for example), symptoms, or disease temporally associated with the use of a medicinal product, whether or not considered related to the medicinal product. TEAEs are defined as AEs with start or worsening date during the on-treatment period (from the first dose date of trial intervention to 50 days after the last dose date of trial intervention, unless otherwise specified in the applicable sub-study protocol). 3.
