2 臨床研究の目的及び内容並びにこれに用いる医薬品等の概要

(1)臨床研究の目的及び内容

がん患者における、がんに起因する咳嗽による苦痛症状に対して、ガバペンチンの投与の有効性と安全性を探索的に評価し、将来の検証的試験へ進むことの妥当性を判断するとともに、その試験デザインの検討に資する基礎的データを得ることを目的とする。

以下の基準を全て満たす患者とする。
(1) 組織学的又は細胞学的又は臨床的にがんと診断された患者
(2) 臨床医の判断でがんに由来する咳嗽と判断される患者
(3) 咳嗽に関するNRSが4以上である患者
(4) 同意取得時に18歳以上の患者
(5) ECOG PSが0-2の患者
(6) 臨床医の判断で生命予後が1ヶ月以上見込まれる患者
(7) 試験期間中の症状日誌記載に対して同意している患者
1. Patients with a histological, cytological, or clinical diagnosis of cancer.
2. Patients whose cough is judged by the treating physician to be attributable to cancer.
3. Patients with a cough severity Numerical Rating Scale (NRS) score of 4 or higher.
4. Patients aged 18 years or older at the time of informed consent.
5. Patients with an Eastern Cooperative Oncology Group Performance Status (ECOG PS) of 0-2.
6. Patients with an estimated life expectancy of at least 1 month, as judged by the treating physician.
7. Patients who agree to complete a symptom diary throughout the study period.
以下のいずれかに該当する患者は本研究に組み入れないこととする。
(1) ガバペンチノイド(ガバペンチン・プレガバリン・ミロガバリン)使用中の患者
(2) ステロイドを含む化学療法施行中の患者(慢性疾患に対する継続的な内服ステロイドは許容する)
(3) 試験開始前1ヶ月以内に化学療法変更があった患者
(4) 試験期間中に化学療法変更予定がある患者
(5) 咳嗽の原因として他の疾患(喘息/COPD/GERD/肺炎/ACE阻害剤など)に由来すると判断される患者
(6) 内服が不可能な患者
(7) 認知症やせん妄などによる認知機能障害によって症状の評価が困難な患者
(8) ガバペンチノイド(ガバペンチン・プレガバリン・ミロガバリン)に対し過敏症の既往歴のある患者
(9) 登録前直近の2週間以内の血液検査において肝機能低下(AST, ALT施設基準上限の3倍以上、T-Bil施設基準上限以上)のある患者
(10) 登録前直近の2週間以内の血液検査において腎機能低下(GFR60mL/min未満)のある患者
(11) 現在喫煙習慣のある患者
(12) アルコール依存のある患者
(13) 妊婦、授乳婦、妊娠する可能性があり試験期間中に避妊する意思のない患者
(14) 研究責任者/分担者がこの研究への参加を不適当と判断した患者
1. Patients currently receiving a gabapentinoid (gabapentin, pregabalin, or mirogabalin).
2. Patients receiving a chemotherapy regimen that includes corticosteroids. Continued oral corticosteroid therapy for chronic conditions is permitted.
3. Patients whose chemotherapy regimen has been changed within 1 month before the initiation of study treatment.
4. Patients scheduled to undergo a change in chemotherapy during the study period.
5. Patients whose cough is judged to be attributable to a cause other than cancer, such as asthma, chronic obstructive pulmonary disease (COPD), gastroesophageal reflux disease (GERD), pneumonia, or the use of an angiotensin-converting enzyme (ACE) inhibitor.
6. Patients who are unable to take oral medication.
7. Patients whose symptoms cannot be adequately assessed because of cognitive impairment due to dementia, delirium, or other conditions.
8. Patients with a history of hypersensitivity to any gabapentinoid (gabapentin, pregabalin, or mirogabalin).
9. Patients with hepatic dysfunction, defined as aspartate aminotransferase (AST) or alanine aminotransferase (ALT) levels at least 3 times the institutional upper limit of normal, or a total bilirubin level at or above the institutional upper limit of normal, based on the most recent blood test performed within 2 weeks before enrollment.
10. Patients with renal dysfunction, defined as a creatinine clearance of less than 60 mL/min calculated using the Cockcroft-Gault equation, based on the most recent blood test performed within 2 weeks before enrollment.
11. Patients who are current smokers.
12. Patients with alcohol dependence.
13. Pregnant or breastfeeding women, or women of childbearing potential who are unwilling to use contraception during the study period.
14. Patients considered unsuitable for participation in the study by the principal investigator or a subinvestigator.

本試験では、がんに由来する咳嗽に対するガバペンチンの用量調整プロトコルに基づいた投与をプロトコル治療と定義する。ガバペン錠を試験薬と定義する。
使用薬剤:ガバペン錠
治療法:試験期間中は咳嗽の症状の強さと副作用の評価を継続して行う。研究対象者が現時点より咳嗽症状の改善を希望し、かつ研究対象者が許容できない有害事象がないと判断した場合には2日以上空けて一段階ずつ増量を行う(最大1200mg)。研究対象者が許容できない有害事象と判断した場合には、一段階減量もしくは最小用量であれば中止する。一度でも減量を行なった場合には以降の増量は行わない。14日目までの治療を完遂した後の投与継続の可否については担当医の臨床判断に委ねるが、中止する際には段階的な漸減を行うこととする。
※ガバペンチン200mg/錠 内服方法
STEP0: 内服終了(以降、再開しない)
STEP1 (400mg/日): 朝食後1錠、眠前1錠
STEP2 (800mg/日): 朝食後1錠、昼食後1錠、眠前2錠
STEP3 (1200mg/日): 朝食後2錠、昼食後2錠、眠前2錠
In this study, protocol treatment is defined as the administration of gabapentin in accordance with the dose-adjustment protocol for cancer-related cough. GABAPEN tablets are defined as the study drug.

Study drug: GABAPEN tablets

Treatment regimen: Cough severity and adverse events will be assessed continuously throughout the study period. If the participant wishes to achieve further improvement in cough symptoms and has no adverse events that they consider intolerable, the dose will be increased by one step at a time, with an interval of at least 2 days between dose escalations, up to a maximum dose of 1,200 mg/day.

If the participant experiences an adverse event that they consider intolerable, the dose will be reduced by one step. If the participant is already receiving the minimum dose, treatment will be discontinued. Once the dose has been reduced, no subsequent dose escalation will be permitted.

After completing protocol treatment through Day 14, whether gabapentin is continued will be determined by the treating physician based on clinical judgment. If gabapentin is discontinued, the dose must be tapered gradually.

Dosing schedule for GABAPEN 200-mg tablets
* STEP 0: Treatment discontinued; treatment must not be restarted thereafter.
* STEP 1 (400 mg/day): One tablet after breakfast and one tablet at bedtime.
* STEP 2 (800 mg/day): One tablet after breakfast, one tablet after lunch, and two tablets at bedtime.
* STEP 3 (1,200 mg/day): Two tablets after breakfast, two tablets after lunch, and two tablets at bedtime.

レスター咳問診票 日本語版
登録時および14日目に評価を行う。
咳嗽に関するQOL評価基準として臨床研究で一般的に用いられているレスター咳問診票を使用する。レスター咳問診票は身体面8項目・精神面7項目・社会面4項目の計19項目の質問項目があり、各質問に対してそれぞれ1〜7点の7段階リッカート尺度を用いて回答する。各ドメインにおける平均スコアの合計(3〜21点)を合計スコアとして評価する(高得点ほど高評価となる)。
合計スコア1.3点以上の変化をMCIDとみなす。
Japanese Version of the Leicester Cough Questionnaire (LCQ)

Assessments will be performed at enrollment and on Day 14.

The Leicester Cough Questionnaire, which is widely used in clinical research to assess cough-related quality of life (QOL), will be used. The questionnaire consists of 19 items across three domains: 8 items in the physical domain, 7 items in the psychological domain, and 4 items in the social domain. Each item is rated on a 7-point Likert scale ranging from 1 to 7.

The total score is calculated by summing the mean scores of the three domains and ranges from 3 to 21, with higher scores indicating better cough-related QOL.

A change of 1.3 points or greater in the total score will be considered the minimal clinically important difference (MCID).
(1) 咳嗽のNRS
登録時から14日目まで連日日誌に記載する。
NRSは主観的重症度評価であり、0〜10の数字で評価する。0を「全く苦痛がない」、10を「想像しうる最大の苦痛」と定義し、過去24時間の平均的な咳のつらさを1つの整数で回答させる。咳嗽に関する研究ではVisual Analogue Scale (VAS)が用いられることが多く、MCIDは30mm以上の変化である。本研究ではVASの評価の煩雑さから、同等に有用な尺度であるNRSを用いる。咳嗽に関するNRSに固有のMCIDは確立していないが本研究ではVASに準じて3以上の改善をMCIDと定義する。

(2) 改善に関する包括的な印象Global impression
PGI-I(Patient Global Impression - Improvement)は、治療を受けた後の全体的な症状の変化(改善・悪化)に対する患者自身の印象を、1(非常に改善した)から7(非常に悪化した)の7段階で評価する。

(3) 各用量におけるレスター咳問診票の変化量
プロトコル治療完了時に内服していたガバペンチンの用量ごとにレスター咳問診票の変化量を評価する。

(4) 最大用量まで増量できた割合

(6) 咳嗽に対するオピオイドレスキュー使用回数の変化

(7) 併用治療薬
試験期間中に使用されたオピオイドおよびその他の鎮咳剤の使用状況を評価する。具体的には、使用薬剤名、用法・用量、使用開始・終了日、目的(例:咳嗽/疼痛)を症例報告書に記録し、日ごとの内服状況を確認する。評価期間は試験薬初回投与日からプロトコル治療終了日までとする(収集対象とする薬剤リストは6.3.3に記載のオピオイドおよびその他の鎮咳剤とする)。

(8) 有害事象
有害事象の重症度評価はCTCAE Ver.6.0日本語訳JCOG版を用いる。
1. Cough Numerical Rating Scale (NRS)
Participants will record their cough NRS score daily in the symptom diary from enrollment through Day 14.

The NRS is a subjective measure of symptom severity rated on a scale from 0 to 10. A score of 0 is defined as no distress at all, and a score of 10 as the worst distress imaginable. Participants will select a single integer representing the average distress caused by their cough during the preceding 24 hours.

The Visual Analogue Scale (VAS) is commonly used in cough research, and a change of at least 30 mm is regarded as the minimal clinically important difference (MCID). In this study, the NRS, which has comparable utility, will be used because it is more practical to administer than the VAS. Although a cough-specific MCID has not been established for the NRS, an improvement of at least 3 points will be defined as the MCID in this study, in accordance with the VAS threshold.

2. Patient Global Impression of Improvement (PGI-I)
The Patient Global Impression of Improvement (PGI-I) assesses the participant's overall impression of the change in symptoms, including improvement or worsening, following treatment. It is rated on a 7-point scale ranging from 1 (very much better) to 7 (very much worse).

3. Change in the Leicester Cough Questionnaire Score by Gabapentin Dose
The change in the Leicester Cough Questionnaire score will be evaluated according to the dose of gabapentin being taken at the completion of protocol treatment.

7. Concomitant Medications
The use of opioids and other antitussive medications during the study period will be evaluated. Specifically, the name of each medication, dosage and administration, start and end dates, and indication for use (e.g., cough or pain) will be recorded in the case report form, and daily medication use will be assessed.

The evaluation period will extend from the date of the first dose of the study drug through the date of completion of protocol treatment. The medications to be collected are the opioids and other antitussive medications listed in Section 6.3.3.

8. Adverse Events
The severity of adverse events will be graded using the Japanese Clinical Oncology Group (JCOG) Japanese translation of the Common Terminology Criteria for Adverse Events (CTCAE), Version 6.0.
