1.肩関節脱臼、上腕骨骨折、鎖骨骨折、神経損傷などの整形学的疾患歴がある人 2.上肢の手術歴がある人 3.計測動作で肩関節に痛みが出る人 4.過去にテーピングを行ったり、ゴム製品を使ったことで皮膚に発赤や痒み、発疹、腫脹あるいはアレルギー反応(粘膜・呼吸器症状、血圧低下など)が出たことがある人 5.上半身の露出に抵抗がある人 6.本研究の同意が得られない人 |
1.Individuals with a history of shoulder dislocation, humeral fracture, clavicular fracture, or nerve injury. 2.Individuals with a history of surgery on the upper limbs. 3.Individuals experiencing shoulder pain at the time of measurement. 4.Individuals who have previously experienced skin redness, itching, rashes, swelling, or allergic reactions (such as symptoms affecting mucous membranes or the respiratory tract, or a drop in blood pressure) due to the use of athletic tape or rubber products. 5.Individuals who are uncomfortable exposing their upper body 6.Individuals who do not provide consent for this study. |

本研究では、肩甲骨外旋および後傾を誘導するテーピングが、投球近似肢位における肩甲骨周囲筋の筋活動および肩甲骨角度に及ぼす影響を探索的に検討するため、2種類のテープ貼付条件を介入として設定する。 1.キネシオロジーテープ貼付条件 2.非伸縮性テープ貼付条件(プラセボ) 本研究はクロスオーバーデザインを採用する。対象者は18歳以上40歳未満の健常成人男性とし、ポスター掲示による公募で募集する。対象者数は10名(20肩)とする。第1期の計測では、対象者を無作為にキネシオロジーテープ貼付群(5名10肩)と非伸縮性テープ貼付群(5名10肩)に割り付ける。 第1期終了後、1週間以上のウォッシュアウト期間を設け、第2期の計測を実施する。第2期では、第1期とは逆のテープ条件を適用する。各期の計測開始時にはベースライン測定としてテープを貼付せずに計測を行い、終了時にはテープ貼付状態で計測を行う。 本研究では上衣は着用せず、上半身を露出した状態で行う。そのため、研究対象者のプライバシーや羞恥心に配慮するため、計測環境周囲にパーテーションを置く。本研究の動作課題は両肩関節で行うが、テープの貼付は一側ずつ行い、貼付および動作課題は利き手側から行う。利き手側での計測が終了したら、非利き手側でのテープの貼付を行い、再度計測を行う。 動作課題は以下の3種類とする。 1.肩関節外転位で内外旋中間位を保持する課題 2.1の肢位から肩関節外旋90度までの運動 3.1の肢位から肩関節最大外旋運動 動作課題時には3キログラムの重錘を測定側手関節に装着する。各動作課題を行う際の保持時間および速度について、動作課題1は5秒間の保持、動作課題2および3はそれぞれ5秒間かけて行う。動作課題は計9回行う(動作課題1を3回、動作課題2を3回、動作課題3を3回)。すべての動作課題は座位で体幹および骨盤を正中位に保持した状態で行う。 介入条件としてキネシオロジーテープ(Kinesio Tex Wave、KINESIO ASIA社)、プラセボ条件として非伸縮性テープ(バトルウィン、NICHIBAN社)を使用する。いずれのテープも、烏口突起から第12胸椎棘突起へ向かう走行で貼付する。貼付時のテープ伸張率は50パーセントに統一し、剥離紙付きテープ長は以下の回帰式に基づいて算出する。 剥離紙付きテープ長 = 0.77 × 烏口突起と第12胸椎棘突起間距離 + 0.65 貼付時には、被験者の肩関節を最大水平内転位とし、肩甲骨周囲の皮膚が伸張された状態を作ったうえで貼付することで、貼付条件の再現性を確保する。 肩甲骨角度および肩甲上腕関節角度の計測には3次元磁気式動作解析装置(Viper、Polhemus社)および解析ソフト(Enobmotion、スリーディー社)を使用する。サンプリング周波数は960ヘルツとする。磁気センサーは胸骨柄、肩峰、三角筋粗面遠位部の皮膚表面に貼付する。センサー装着後、スタイラスに取り付けた第4センサーを用いて以下の解剖学的ランドマークをデジタイズする。第7頸椎棘突起、第8胸椎、胸骨切痕、剣状突起、肩甲棘基部、肩甲骨下角、肩峰後外側部、上腕骨内側上顆および外側上顆。デジタイズはすべて同一研究者が実施し、測定誤差を最小化する。取得したランドマーク情報を基に胸郭、肩甲骨、上腕骨の3次元座標系を構築し、国際バイオメカニクス学会の肩関節推奨法に従って肩甲骨および肩甲上腕関節の3次元角度を算出する。 肩甲骨周囲筋の筋活動データは表面筋電計(Ts myo、トランクソリューション社)を用いて収集する。サンプリング周波数は1000ヘルツとする。被験筋は僧帽筋上部、僧帽筋中部、僧帽筋下部、前鋸筋とする。電極貼付位置は、僧帽筋上部は第7頸椎棘突起と肩峰の中点、僧帽筋中部は第3胸椎棘突起と肩甲棘基部の中点、僧帽筋下部は第8胸椎棘突起と肩甲棘基部を結ぶラインの近位3分の2、前鋸筋は肩甲骨下角の高さにおける腋窩中線上とする。 各動作課題での筋活動測定後、Ts myoの解析機能を用いて筋電波形を処理する。原波形は50から450ヘルツを透過するバンドパスフィルタを適用し、ルートミーンスクエア法で平滑化する。各動作課題中の筋活動値をサンプリングデータとし、筋電図積分値を算出する。算出した筋電図積分値を、各筋の等尺性筋力発揮時に得られた筋電図積分値で除すことで正規化を行う。 |
This study aims to exploratively examine the effects of taping that induces scapular external rotation and posterior tilt on scapular muscle activity and scapular angles in a throwing-like arm position. Two taping conditions are set as interventions. 1.Kinesiology tape condition 2.Non-elastic tape condition (placebo) This study adopts a crossover design. Participants will be healthy adult males aged 18 to under 40, recruited through poster advertisement. The total number of participants will be 10 (20 shoulders). In Phase 1, participants will be randomly allocated to the kinesiology tape group (5 participants, 10 shoulders) or the non-elastic tape group (5 participants, 10 shoulders). After Phase 1, a washout period of at least 1 week will be provided, followed by Phase 2. In Phase 2, the opposite taping condition from Phase 1 will be applied. At the beginning of each phase, baseline measurements will be performed without tape, and at the end of each phase, measurements will be performed with tape applied. In this study, participants will not wear an upper-body garment, leaving the upper body exposed; therefore, partitions will be placed around the measurement area to respect the participants' privacy and prevent embarrassment. Although the movement tasks involve both shoulder joints, the tape will be applied to one side at a time, starting with the dominant side. Once measurements on the dominant side are complete, the tape will be applied to the non-dominant side, and measurements will be repeated. Three movement tasks will be performed. 1.Maintaining shoulder abduction with the humerus in a neutral rotation position 2.Shoulder external rotation to 90 degrees from the position of Task 1 3.Maximal shoulder external rotation from the position of Task 1 During the movement tasks, a 3-kilogram weight will be attached to the wrist of the measured arm. Regarding the hold time and speed for each movement task, Task 1 involves a 5-second hold, while Tasks 2 and 3 are each performed over a duration of 5 seconds. A total of nine trials are conducted (three trials each for Tasks 1, 2, and 3).All movement tasks will be performed in a seated position with the trunk and pelvis in a neutral posture. For the intervention condition, kinesiology tape (Kinesio Tex Wave, KINESIO ASIA) will be used, and for the placebo condition, non-elastic tape (Battlewin, NICHIBAN) will be used. Both types of tape will be applied along a path from the coracoid process toward the spinous process of the twelfth thoracic vertebra. The tape stretch rate will be standardized at 50 percent, and the tape length with backing paper will be calculated using the following regression formula: Tape length = 0.77 x distance between the coracoid process and the twelfth thoracic vertebra + 0.65 During application, the participant's shoulder will be positioned in maximal horizontal adduction to create skin tension around the scapula, ensuring reproducibility of the taping condition. Scapular angles and glenohumeral angles will be measured using a three-dimensional magnetic motion analysis system (Viper, Polhemus) and analysis software (Enobmotion, Three-D). The sampling frequency will be set at 960 hertz. Magnetic sensors will be attached to the skin over the manubrium, acromion, and distal deltoid region. After sensor attachment, a fourth sensor mounted on a stylus will be used to digitize the following anatomical landmarks: spinous process of the seventh cervical vertebra, eighth thoracic vertebra, sternal notch, xiphoid process, root of the scapular spine, inferior angle of the scapula, postero-lateral acromion, medial and lateral epicondyles of the humerus. All digitizing will be performed by the same researcher to minimize measurement error. Based on the digitized landmarks, three-dimensional coordinate systems for the thorax, scapula, and humerus will be constructed, and joint angles will be calculated according to the International Society of Biomechanics Shoulder Recommendations. Muscle activity of the scapular muscles will be recorded using a surface electromyography system (Ts-myo, Trunk Solutions). The sampling frequency will be set at 1000 hertz. The target muscles will be the upper trapezius, middle trapezius, lower trapezius, and serratus anterior. Electrode placement will be as follows: upper trapezius at the midpoint between the spinous process of the seventh cervical vertebra and the acromion; middle trapezius at the midpoint between the spinous process of the third thoracic vertebra and the root of the scapular spine; lower trapezius at two-thirds of the line between the spinous process of the eighth thoracic vertebra and the root of the scapular spine; serratus anterior at the mid-axillary line at the height of the inferior angle of the scapula. After measuring muscle activity during each movement task, EMG signals will be processed using the analysis functions of Ts-myo. Raw EMG signals will be band-pass filtered from 50 to 450 hertz and smoothed using the root mean square method. Integrated EMG values during each movement task will be calculated from the sampled data. The integrated EMG values will be normalized by dividing them by the integrated EMG values obtained during isometric contractions of each muscle. |
