Key inclusion & exclusion criteria	Inclusion Criteria	(i) Patients admitted to an acute care hospital for heart failure (ii) Patients whose symptoms have improved to the point where discharge is imminent (iii) Patients with heart failure AHA staging Stage C or higher.
	Exclusion Criteria	Patients with psychiatric disorders such as depression and schizophrenia, and patients diagnosed with delirium.
